Documentation
You will need to provide our office with documentation as part of the accommodation process. This information can serve as a support tool in identifying reasonable academic accommodations and will be stored securely within Student Access Services.
This documentation must be from someone who is qualified to diagnose or help you manage your disability and should not be from someone who has a personal relationship with you.
Proper documentation should contain all of the following things:
- Your first and last name
- Your date of birth
- Rose State Student ID Number (optional)
- The date the documentation was prepared or written
- The first and last name of the person who prepared the documentation
- The specific name of your disability, for example: ADD/ADHD, Learning Disability – Written Expression, Seizure Disorder, Bipolar Disorder, etc.
- A description of how this disability affects your daily life
- The projected progression or stability of your disability
You may submit your documentation electronically on the form or by emailing studentaccessservices@rose.edu.
