Pregnancy Accommodations

This form allows students who are pregnant, recently gave birth, or experiencing pregnancy-related conditions to request academic adjustments under Title IX. You do NOT need a doctor's note to submit this form.

By signing below, I acknowledge that I understand that this form notifies the Title IX Office that I am requesting pregnancy/parenting-related academic adjustments. I understand that my instructors will receive a notice explaining their responsibilities under Title IX, but my specific medical details will not be shared.

After submitting this form, the Title IX Office / Student Access Services will contact you and send appropriate notifications to your instructors.
